Personalized Sports Physicals
Kentucky High School Athletic Association (KHSAA) and Indiana High School Athletic Association (IHSAA) require all student-athletes to have an up-to-date physical for participation in 2025-2026 sports.
Make sure that your student-athlete age (middle school and above) is ready for competition by completing their annual KHSAA or IHSAA approved sports physical. St. Elizabeth Physicians recommends that all student-athletes schedule their sports physical with their family or primary care physician.
If you are unable to see your primary care physicians, St. Elizabeth Sports Medicine provide a $25 comprehensive sports physical. To make an appointment please contact their office at the number below:

All athletes need to bring a fully completed KHSAA or IHSAA sports physical form, complete with a parent/guardian signature if the athlete is under the age of 18. If you do not have one, please choose from one of the forms below.
