Ambulatory Care Management

We’re part of your primary care team.

Our ambulatory care management team is committed to developing and coordinating a health and wellness plan that is personalized just for you.

What is Care Management?

The St. Elizabeth Physicians ambulatory care management department is interdisciplinary and places you, the patient,at the center of your care team. Our holistic model of care consists of individuals with diverse skill sets who are dedicated to caring for you and connecting you with the resources you need to meet your physical and emotional wellness goals.

All of our St. Elizabeth Physicians primary care offices have a care management team member on staff to assist with appropriate screenings and assessments needed to develop and implement a personalized plan of care. 

Who is on my care management team? 

Your care team consists of individuals dedicated to getting you the care you need –when you need it. This includes nurse care coordinators, social workers, respiratory therapists, outreach specialists and dietitians.

How can the care management team help me?

The care management team works with you to:

  • Answer and understand any primary care questions or concerns
  • Coordinate on-going care after an unexpected illness or hospitalization
  • Provide support for managing chronic conditions, such as COPD, heart failure, diabetes, renal disease, etc.
  • Provide short-term same day access to counseling for emotional needs
  • Offer assistance if you are unable to afford your medications
  • Coordinate reliable transportation to and from appointments
  • Connect with community partners for resource tools and information

Advance Care Planning (ACP) is a process for setting goals and planning now for the healthcare you would want if you could no longer speak for yourself. Your care team is right here to help you and your family develop a care plan that meets your wishes and preferences.
