- Stay up to date on recommended screenings: MyChart’s preventive care feature tracks recommended vaccines and procedures based on your age and health. It will even send reminders if you’re overdue for any preventive care measures.

- Monitor your health: Track your health by comparing current records with previous exams, labs and visits.

- Waitlist feature: Get notified if an earlier appointment becomes available. You must already have an appointment scheduled to use this feature.
- Notifications: Receive appointment reminders prior to your visit.
- Connect your MyChart accounts: If you have a MyChart account with other health care organizations, you can connect them to your St. Elizabeth MyChart account. This will allow you to get a comprehensive view of all your health information – in one place.
- Keep track of your family’s health: With proxy access, you can stay on top of everyone’s appointments and check in on family members who need extra help, all from your MyChart account.
- Need a copy of your medical records? We take great care to ensure your privacy and to keep your medical records safe and secure. If needed, rather than having your medical records mailed to you in paper form, you can submit a request to have these documents electronically uploaded into MyChart at no charge.
Getting Started
To sign up for MyChart, you’ll need an activation code, which can be obtained in one of three ways:
1. Request a code online at MyChart – Request an Activation Code
2. Ask a staff member to help you sign up for MyChart at your next appointment or hospital visit.
3. An activation code is located at the bottom of the After Visit Summary you receive following your visit at
any St. Elizabeth location.
Once you receive an activation code, go to the MyChart signup page to create your account.

Stay connected to your health information on-the-go by downloading the MyChart mobile app. This secure app lets you take advantage of MyChart features from anywhere, anytime.
The MyChart app is easy to navigate and even easier to set up:
1. Download the MyChart app from the App Store (iOS) or Google Play (Android) to your smartphone or tablet.
2. Open the app and accept the Terms and Conditions.
3. Scroll through the list and click on “St. Elizabeth Healthcare.”
